NHIC has discontinued issuing UPINs effective June 29, 2007. Additionally, CMS has
extended its UPIN public “look-up” functionality and Registry website until May 23,
2008. Under the Medicare FFS contingency plan, UPINs and surrogate UPINs may still be
used to identify ordering and referring providers and suppliers until further notice.

Effective May 15, 2006 UPINs will be assigned only to physicians and the
following non-physician practitioners: Nurse Practitioners (NPs),
Clinical Nurse Specialists (CNSs), and Physician Assistants (PAs),
and Certified Nurse Midwives (CNM)
